named supervisor with enough context to act, and does not escalate
further on its own.
Trigger a major-classified violation; confirm it escalates through the
standard HR escalation path (not a bespoke agent-only path) and produces
a full audit trail entry.
Reassign an Agent Worker's supervisor; confirm in-flight
minor-deviation notifications route to the new supervisor, not the
outgoing one.
Attempt to classify a violation without the taxonomy in place (i.e. test
this before the P0 TODO closes); confirm the system fails safe (e.g.
treats an unclassifiable event as requiring human triage) rather than
guessing a severity.
